Request a certificate of insurance
For current clients. Tell us who needs the certificate and where to send it.
During business hours (Mon–Fri, 8 AM–6 PM ET), you’ll get your certificate back within 30 minutes. Requests sent after hours go out first thing the next business morning.
Certificates are free for our clients. We send it straight to the holder and copy you.
Email us your request
Email is the easiest way. Send it to certificates@pharosins.com and include:
- Your business name and USDOT or MC number
- The certificate holder’s name and full mailing address
- The email(s) to send it to. You can list more than one email.
- When you need it
Most delays come from a missing or partial holder address.
Additional insured or waiver of subrogation? Attach the holder’s requirements and we’ll check your policy first.
The button opens an email with these lines ready to fill in.
Need one again? Just reply to your last certificate email.
Prefer a form? Use the one below. It asks for the same details.
A certificate isn’t a federal or state filing. For the filings that keep your authority active, see Filings and certificates.
We check every request against your policy, and some wording needs an approved endorsement. A certificate doesn’t change, extend or add coverage, or give anyone rights the policy doesn’t provide.
